Terms and conditions for Digital Library Membership

Recording link will be sent to you monthly as well as any password information needed to access. Equipment will be mailed to new clients. Recordings membership does not include any live classes, virtual or in person. Membership is non refundable and non transferrable.

FIT4MOM OnDemand Monthly Subscription

With your paid monthly membership you are given free access to FIT4MOM OnDemand. You will have unlimited access to OnDemand workouts. When your FIT4MOM membership is properly cancelled according to the Terms and Conditions, your OnDemand subscription will be automatically cancelled. If you would like to keep your OnDemand subscription you can re-enroll using the same login information and purchase your desired paid subscription.

FIT4MOM MONTHLY MEMBERSHIP AGREEMENT
This contract is made between the client and FIT4MOM North Shore Chicago (MA Strong LLC) and is a month to month agreement and except as provided in the initial 10 day cancellation policy below, may be terminated by either party with the proper advance notice. This membership allows you to attend unlimited drop-in classes a month.

AUTHORIZATION FOR PAYMENT
I authorize Fit4Mom Central North Shore Chicago to initiate a charge to the account entered into Pike13 for my dues. This authorization is to remain effective until I have cancelled my membership through the Fit4Mom Central North Shore Chicago administrator.

I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with Fit4Mom to fulfill my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on file on the day of the month when the membership was started for the first month and then on the 1st of the month going forward. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a late fee. Members are responsible for updating their credit card information online. Members may not attend class who have unpaid balances. If a membership remains unpaid it will be cancelled.

CANCELLATION POLICY
To cancel this membership, the member must contact the Fit4Mom Chicago administrator 15 days prior to their next payment due date to request a cancellation. No refunds will be given for dues once it has been paid. This membership is not transferable to another person or anywhere outside the Central North Shore Chicago franchise. Once a membership is cancelled, if the member wants to re-enroll they will be charged half the normal registration fee.

HOLD POLICY
Memberships may be held a maximum of 4 months per hold period. Hold starts will coincide with the next payment date of the first of the month and notice must be given 15 days in advance to hold. When a hold period is ended, the monthly auto payments will resume on the return date. There will be a $20 processing fee charged to your credit card on file for the hold period. This fee will be credited to your account upon your return. The only exception to this hold policy are those who become injured and expectant moms who deliver and plan to return to class after recovery. To hold, please email angelabecker@fit4mom.com

MEMBERSHIP DOWNGRADE POLICY

Memberships may be changed with 15 days notice from the billing date. There is no fee to change memberships and no new 3 payment commitment. To change memberships, please email angelabecker@fit4mom.com

INITIAL 10-DAY CANCELLATION POLICY
You, the buyer, may cancel this agreement at any time prior to midnight of the tenth (10th) calendar day after you have signed this agreement. To cancel this agreement written notice must be received by Fit4Mom within the initial ten days. You are responsible for paying for the classes already attended.

CLASS REGISTRATION AND CANCELLATION POLICY

Please register if you plan to attend class and cancel your registration up to 1 hour before class. Cancellations less than 1 hour before class and no shows will be charged a late cancel fee of up to $10.

AGREEMENT
By purchasing this contract, you are agreeing to the terms of this contract. Terms of this agreement may change with proper advance written notice.
